How do I become a new patient?
We are currently accepting new patients. Please find the steps on how to schedule an appointment in our New Patient Tab
Where is Star Family Health and Wellness located?
We are conveniently located on State Street in the heart of Star Idaho. You can access the clinic easily from both the Middleton/Caldwell and Eagle areas.
How soon can I get in for an appointment?
Currently we are able to schedule appointments within 1-5 days. Do not waste precious time waiting weeks to see a provider. Give us a call today!
How should I prepare for my first visit?
Upon scheduling your appointment you will receive a text message from our automated system to fill out your new patient packet. The packet will include; basic demographic information, acknowledgement of our consent forms, and a section to input past social, family, and medical history.
It takes about 15 minutes to complete the paperwork. We encourage you to complete the paperwork prior to your appointment so you can spend less time in the waiting room and more time with your provider!
If you do not have access to technology to complete the paperwork we ask that you arrive to your appointment 15 minutes early.
What do I need to bring with me to my first visit?
Please bring your ID card, Insurance card, and a list of your current medications.
Do you offer telemedicine?
We offer telemedicine that can easily be accessed from your smartphone or computer. If you are interested in learning more about what appointments can be seen via telemedicine, please call the clinic at 208-286-9471.
What is your cancellation policy?
We ask that if you need to cancel your appointment you give the clinic a call with 24 hours notice. If you fail to give proper notice for cancelling your appointment, we will access you a $25 late cancellation fee. We will assess you a $25 no-show fee if you no-show an appointment more than once.
What are my options if Star Family Health and Wellness is out of network or if I do not have insurance.
We offer a 20% discount to patients who are uninsured if the charges are paid in full on the date of service. We are also able to set up a payment plan based on certain situations. Please call the clinic to find out more information, we would love to make something work for you.

How do I create a patient portal account?
You can create a portal account yourself by going to our patient portal tab on the home page. You will then click the “sign up” button to create an account. Once your account is created you can request an appointment and a member of our staff will give you a call to schedule.
What can I do on the patient portal?
The patient portal is our favorite piece of technology! It allows us to maintain a close relationship and line of communication with our patients in a streamlined fashion. Medical records are securely uploaded to the cloud and made accessible to our patients. Not only are you able to access your records with a click of a button, but you are also able to reach a provider directly though secure messaging, keep track of your appointments, request prescription refills, and even order supplements. This direct communication with your provider means you can ask questions and get answers without waiting on someone to call you back!
